STUDENT INFORMATION FORM
Click on the link above (or right-click and choose Save As, or Save Link As) to download the Student Information Form as a Word document. Type the information directly into the form, print it out and return it to class.
If you have difficulty typing information into the form, print it out and hand write the information. If you have difficulty downloading the form, I will provide a blank form in class.
